2015-2016 School Pre-Participation Physicals

Grades 7–12 Must wear shorts Must bring a completed form signed by a parent, form is available from the school's AD office or at ohsaa.org. Cost $20 per athlete, $10 of each fee is returned to the student's school to purchase sports medicine supplies. No insurance accepted. Cash or check only. Please make checks to Good Samaritan Hospital.
