2014-2015 School/Sports Physicals

Student-athletes entering grades 7–12. No appointment necessary. Students must wear shorts. Students must bring a completed Ohio State Physical Form signed by a parent/guardian. Cost: $20 per athlete. $10 of each fee is returned to the student's school. No insurance accepted. Cash or check only
